First Visit

Thank you for choosing Oasis Dental Spa. If this is your first visit with us, please arrive at least 30 minutes prior to your scheduled appointment to complete all necessary forms (Contact Information, Medical history, HIPAA, Policies). We are a paperless office, so upon your arrival you will be handed an iPad to fill out all necessary forms.

Your initial appointment usually will consist of taking X-rays, an examination and review of your treatment options. Your cleaning could be done or started the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

• Any x-rays taken by a previous dentist, if applicable. Please have these send prior to your appointment so Dr. Segovia has the opportunity to review before she sees you.
• A written list of medications you are presently taking.
• If you have dental insurance, bring your insurance card or insurance Identification. This will save time and allow us to help you process any claims.

IMPORTANT: All patients and companions entering our office will need to fill out the COVID exposure form and email to [email protected] no later than 2 days prior to their appointment. All minor patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, please request that they forward them to our office prior to the day of your appointment. If there is not enough time, please pick them up and bring them to our office. We might not be able to complete diagnosis without them and we will suggest to take new ones to complete the diagnosis which will create an out of pocket expense for you.
