Important Information Regarding COVID-19. PLEASE READ BEFORE COMING TO OUR OFFICE
In recognition of ongoing news reports and the continued spread of the COVID-19 virus, We would like to make you aware of a few changes to our office and in hopes to alleviate some patient concerns, so please read carefully to the end.
While many things have changed in the last year, one thing has remained the same: our commitment to your safety, comfort and customer service. We know that most have concerns about infection control and we want to reassure you that Infection control has always been a top priority for our practice and you may have seen this during your visits to our office.
Our infection control processes are made so that when you receive care, it’s both safe and comfortable. We want to tell you about the infection control procedures we follow in our practice to keep patients and staff safe.
Our office follows Government regulations as well as infection control recommendations made by the American Dental Association (ADA), the U.S. Centers for Disease Control and Prevention (CDC) and the Occupational Safety and Health Administration (OSHA). We follow the activities of these agencies so that we are up-to-date on any new rulings or guidance that may be issued. We do this to make sure that our infection control procedures are current and not only adhere to each agencies’ recommendations, but go even beyond.
You will see some changes when it is time for your next appointment. We have incorporated air purifiers in each treatment room and work four-hand-Dentistrty to control aerosol. We made other changes to help protect our patients and team members. It is all being done out of an abundance of concern for everyones health, including our amazing team. For example:
1-. This is the most important so please read carefully: Per our internal policy, every person walking into our office will be required to use a face covering and complete the COVID-19 Exposure Intake form “BEFORE” entering or office for their appointment. This is a screening process that will enable you to have an appointment with us. If you can easily follow the link below and fill directly or please call our office to screen via phone. Please complete it close to your appointment but no later than 2 days prior. If we do not have this form in our system, your appointment could be cancelled.
2-. Please click the button at the end of this letter or follow this link to access the COVID-19 Exposure Form or call our office to let us know and screen via phone.

We have scheduled patient’s appointments to allow for social distancing, update/complete forms, make future appointments, etc so we kindly ask that you please arrive on time for your appointment. If you are a new patient, please check in 30 min prior to your appointment to complete all the new patient forms. If you are an existing patient please arrive 20 min prior to update your forms. Have your insurance card and ID ready for check-in.
4-. We request only necessary individuals be present in our office. If the patient is an adult, please come alone, If the patient is a minor, only one parent/guardian is advised. We may ask escorts to wait in the car or outside our office. If it is a Minor, please be accessible in case there are changes to the treatment plan that need to be reviewed to continue with the procedure. Example: a caries was bigger than anticipated, now needs a different treatment. escorts will be required a COVID Exposure form as well.
5-. Upon arrival, please check in directly at the front desk to review your COVID-19 exposure intake form. At this point your temperature will be taken.
6-. We request all individuals coming into the office to utilize their own face mask while in our common waiting room.
6-. We have hand sanitizer that we will ask you to use immediately once you enter the office. You will also find some in the reception area and other places in the office for you to use as needed.
7-. We will have limited seating each at 6 feet apart. If you choose to sit, We request you stay seated on the chair you have chosen and do not move to another chair or touch any other unnecessary surfaces.
8-. Staff will be cleaning chairs, the general waiting area, and bathroom routinely. We will continue to implement the highest standards for infection control in our treatment rooms.
9-. Patient portal. Unfortunately, the patient portal created back in May 2020 is no longer in use. We are currently looking into more efficient ways to integrate our software with our patient communications. We will let you know of any update. In the meantime, we will continue to communicate via email, text and phone calls.

While we do not know everything about this virus to date, individuals who are elderly and/or immunocompromised appear to be the most at risk of a severe infection. To ensure your health and safety, and the health and safety of everyone at our practice, please reschedule your appointment if you or a member of your household has a cough, fever, and/or flu-like symptoms. In addition, please reschedule your appointment if you have traveled to any areas within the past 14 days that are at high risk for contracting the coronavirus.
